How do I create a copyright page for my publication?

I have recently published my first book, but now wish to update it with the inclusion of a few pictures and improved layout. I would also like to add a copyright page. Is there a template for this?

This is probably not the right answer and all hell and sundry will post in a few minutes screaming "Noooooooooooooooooooooooo!" but what I did, after a bit of research which led me nowhere, was open a fictional book on my shelf and plagerized the copyright.

Obviously I changed all the details to refer to my book and the date of publication.

Does anyone care to shoot me?

All you need to do is add a single line, either at the front, say on the title page, or at the back (some people prefer the back), that says something on the lines of:

© Author name 2012. (You can get the copyright symbol by either going into the Windows character map and C&P from there, or simply holding down the ALT key and typing 0169.)

You can add the statement "All rights reserved", but it is not necessary.

Nah, I did the same thing because it's more or less the same in any book.
