How do I update my book on kdp?

Go to your 'Bookshelf'. Click on the ellipses (...) on the right side of the book listing you want to edit. Click on the items you want to edit.

Thank you so much! I meant, should I create a new book altogether if the revisions are significant (name changes, lenght, plot changes)?

No, because you don't want to lose the reviews or have two editions of the same book for sale. But you can most certainly revise your book description to boast about the revised and updated content.
