I sent a friend an Amazon gift of a $10 ebook I thought she might like. She asked if I would mind her using it on another book instead, which I don't. But neither of us can work out how to do that--it seems to just send her back to the book I chose. If you don't want to post the answer and help people subvert targeted gifts, I really would appreciate it if you sent the answer to me at veinglory at gmail.com.
Your friend should go to the gift certificate section of her account, then click redeem. Unless she tells Amazon to charge differently, purchases will be deducted from it first until it's depleted. At least, that's how it's always worked for me.

That's the way gifted books have always worked for me (as a recipient). They don't work like a gift certificate, which is the conventional wisdom on these fora. Granted, I haven't received a lot of them, but the few that I have, over the years, have worked just as you described.
